Air Force Forms Team to Investigate Super Tucano Crash

TEMPO.CO, Jakarta - The Indonesian Air Force established an investigation team to determine the cause of the crash of two Super Tucano aircraft that took off from Air Squadron 21 Abdulrachman Saleh Air Base in Malang, East Java, on Thursday, November 16. The tragic incident claimed the lives of four officers.

The team formed by the Air Force Airworthiness, Aviation Safety, and Occupational Center would investigate the causes of the crash through the factors known as 5M (Man, Machine, Medium, Mission, and Management).

The Indonesian Air Force spokesman First Marshal R. Agung Sasongkojati said in a press statement that the team would directly examine the aircraft and go through all procedures, including analyzing the weather conditions at the time of the incident, evaluating all personnel involved in the flight, and exploring other possibilities.

The team would also focus the aircraft's Flight Data Recorder which records flight data, engine data, pilot communication data, and flight video until its last second.

“Hopefully, the investigation will run smoothly so that we can all prevent the same incident from recurring,” Sasongkojati said.

The two EMB-314 Super Tucano aircraft, with tail numbers TT-3103 and TT-3111, crashed during the training mission, killing four crew members. They were Lieutenant Colonel Pnb Sandhra Gunawan (Frontseater) (TT-3111), Colonel Adm Widiono (Backseater) (TT-3111), Major Pnb Yuda A. Seta (Frontseater) (TT-3103), and Colonel Pnb Subhan (Backseater) ( TT-3103).

The two light attack jets took off from the Abdulrachman Saleh Air Base at 10:51 a.m. and lost contact at 11:18 a.m. Thursday. Witnesses reported the planes crashed in the Keduwung area, Pasuruan.
